Grant Opportunity for Practice Change Leaders

Practice Change Leaders for Aging and Health is a national initiative, funded by the John A. Hartford Foundation and Atlantic Philanthropies to develop organizational leaders, committed to transforming care for older adults. The program will make up to ten awards of $45,000 each for its cohort of leaders for the year beginning in January 2018. The project requires matching amounts of $20,000 from the grantee, which can be in the form of cash or in-kind resources.

Leaders participating in the grant complete a self-initiated project aimed at integrating improved care for older adults. The projects are operated within the leader's organization, allowing them to remain at their full-time job during the one-year program.

To be eligible, applicants must hold a leadership role in a healthcare delivery organization, health-related institution, or community-based organization with direct responsibility for care that impacts older adults. The applicant’s home institution may be a health plan, hospital, ambulatory clinic, home health agency, assisted living facility, nursing home, or community-based agency providing health-related services to older adults. Applicants must be senior enough in their organization to have decision-making authority and be able to effect change.

Applications must be submitted no later than Wednesday, August 16.
