2026 Q1 MDH LTC Contact Update

Maryland Department of Health’s Infectious Disease Epidemiology and Outbreak Response Bureau wants to continue supporting you in your efforts to reduce healthcare-associated infections, antimicrobial resistance, and COVID-19. To do this effectively, we need your most updated contact information for your facility.
If any changes need to be made for your facility, please reply with corrections in the body of an email. DO NOT send an excel workbook. Specifically, we need the following information for each member of your infection prevention and control team or any other staff at your facility interested in receiving Maryland state-wide communication for the prevention community:
· Contact name
· Best phone number
· Position title
· If the staff person is new, please include the staff’s state date
· Facility Name
· Facility jurisdiction
If you have done so recently, you can disregard this message.
You do not need to reply if everything is up to date.
Please indicate in the email if the following changes need to be made:
· You do not have the call-in information to the LTC calls on the first Thursday of each month at 12:30
· You would like to be added to an additional MDH contact list (dialysis)
· You would like to be removed from any/all MDH contact list
